Traitement d’un accident vasculaire cérébral par fermeture endovasculaire du foramen ovale perméable
Le foramen ovale perméable (FOP) est un défect embryonnaire interauriculaire potentiellement associé aux accidents vasculaires cérébraux ischémiques (AVCI). L’une des options thérapeutiques consiste en l’occlusion du shunt par voie endovasculaire. Celle-ci est recommandée pour des patients avec un historique d’AVCI.
Nous rapportons ici le cas d’un patient sans histoire d’AVCI et sans profil de risque opératoire particulier soumis à un triple pontage coronarien. En post-opératoire, il développe une hémiplégie gauche avec désaturation O2 persistante. Un FOP avec shunt bidirectionnel et anévrysme du septum inter-auriculaire est alors mis en évidence. Celui-ci est considéré responsable de l’AVCI après avis de deux neurologues. Une procédure de fermeture endovasculaire par mise en place d’une prothèse AmplatzerTM Septal Occluder (Abbott) est réalisée avec succès. Une double anti-agrégation plaquettaire à long-terme est maintenue. Après 6 mois, l’évolution clinique est remarquable et sans complications

Stroke treatment by endovascular septal occlusion of patent foramen ovale
Patent foramen ovale (PFO) is an embryonic interatrial defect potentially associated with ischemic cerebral strokes. One option is the occlusion of the atrial shunt by endovascular means. This procedure is recommended for patients with a history of cryptogenic strokes.
We report here the clinical case of a patient without a history of cryptogenic strokes and without a pre-operatory risk profile that underwent a triple coronary bypass procedure. During post-op, a left hemiplegia with persistent O2 desaturation developed despite anti-platelet treatment. A bidirectional auricular septal shunt then discovered and considered the cause of stroke by two neurologists. Endovascular shunt occlusion then prescribed and performed successfully using an AmplatzerTM Septal Occluder device. The double anti-platelet treatment is maintained at long-term. After 6-month follow-up, the clinical evolution has been excellent without any complications.
