Critères STOPP and START 3e version : une actualisation des critères de prescription appropriée chez la personne âgée
Introduction : A l’heure actuelle, il n’existe pas de guide de recommandation de bonne pratique en matière de déprescription pour les patients âgés poly-médiqués. Cependant, en raison de l’évolution rapide de la recherche clinique et pharmacologique en géria-trie, les critères explicites « Stopp and Start » ont été récemment révisés et validés et fournissent au clinicien une aide à la révision du traitement médicamenteux.
Matériel et méthodes : Une revue détaillée par système a été réalisée par un panel d’experts européens sous forme d’étude Delphi, en enrichissant les critères de la version précédente par de nouveaux critères de pres-cription potentiellement inappropriée suggérant l’arrêt de médicaments (critères STOPP) ou d’omission de prescription suggérant d’initier certains traitements (critères START). Les critères jugés obsolètes selon les nouvelles données de la littérature ont été retirés.
Résultats : Centre-trente-trois critères STOPP et 57 critères START ont été validés. Comparé à la version antérieure, la version actuelle s’est enrichie de plus de deux tiers de nouveaux critères, reflétant l’impor-tante avancée de la prise en charge thérapeutique des patients âgés polymorbides et fragiles. Le système cardio-vasculaire est celui qui connait le plus d’évolu-tion, avec entre autres l’initiation de nombreux traite-ments dans l’insuffisance cardiaque.
Conclusion : La version actualisée des critères STOPP and START offre au clinicien un outil pratique de détec-tion de la prescription appropriée ou inappropriée pour les personnes âgées, en attirant leur attention, système par système, sur les prescriptions potentiel-lement inappropriées et les omissions de traitement à potentiellement initier.

STOPP and START criteria 3rd version: updated criteria for appropriate prescribing in older people
Introduction : At present, there are no best-practice guidelines for deprescribing drugs for elderly patients with multiple medications. However, due to the rapid evolution of clinical and pharmacological research in geriatrics, the explicit «Stopp and Start» criteria for drug review of geriatric prescribing have recently been revised and validated and provide the clinician with an aid to revise drug therapy.
Material and methods : A detailed review by system was carried out by a panel of European experts in the form of a Delphi study, enriching the criteria of the previous version with new criteria for potentially inappropriate prescribing suggesting the discontinu-ation of drugs (STOPP criteria) or omitted prescribing suggesting the initiation of some treatments (START criteria). Criteria deemed obsolete according to new data in the literature were removed.
Results : Thirty-three STOPP criteria and 57 START criteria were validated. Compared with the previous version, the current version has been enriched by more than two-thirds with new criteria, reflecting the major advances in the therapeutic management of polymorbid older patients with a frailty profile. The cardiovascular system has undergone the most devel-opment, particularly in terms of the treatments to be initiated for heart failure.
Conclusion : The updated version of the STOPP and START criteria provides clinicians with a practical tool for appropriate prescribing in the elderly, by drawing their attention, system by system, to potentially inap-propriate prescriptions and omissions of treatment that could potentially be initiated.
